Subject:      new macro %sasindex added

I have added a new macro %sasindex that helps with sas program documentation. It reads the program headers in a directory and creates an index with the program name matched to the program purpose. It is highly dependent on your standard header. The version I have written matches my own standard header but it is easy to amend it to suit your own.
